Complex care and outreach packages
Care UK specialises in delivering high level care to individuals with very challenging and complex needs, both mental and physical. These packages of care are arranged as part of a multi-agency approach and cover all elements of the service from specialist equipment to specialist staff input. This may include establishing an intensive one-to-one package. We are able to offer this service to people in their own home as an outreach service or, if this is not possible, the service will be provided within one of our care homes. Complex care packages offer a cost-effective solution for difficult to place individuals, enabling them to be cared for outside hospital and in a more appropriate setting, in their area.
