Home from hospital scheme
Care UK operates a number of home from hospital schemes nationwide, providing time limited care in the home for patients returning home from hospital after illness, surgery or accident. The emphasis is strongly placed on people achieving full rehabilitation and regaining independence whilst also enabling quicker discharge from hospital.
Many service users are fit to return home, but in the early days after discharge from hospital they need some care in the home to regain their confidence and strength. These Home from Hospital care services provide personal care and/or any other practical assistance for a two to six week period. At the end of this period, the service user is usually able to remain at home without any further help or assistance.
