Intermediate care and rehabilitation
We operate several intermediate care schemes in partnership with PCTs and local authorities. These provide nursing care and focused rehabilitation for patients recovering from a recent illness, injury or surgery who no longer require an acute hospital bed but are not yet ready to return home. These rehabilitation and enablement programmes are delivered by specially trained multi-disciplinary teams. They provide the support to progress towards optimal independence and assist a safe return to the individual’s home. The services are specifically designed to support early discharge from hospital beds, prevent inappropriate admission to acute inpatient care and premature admission to long term care.
