Our supported living services enable service users to choose how they live, where they live, with whom they live, who supports them and how they are supported.
Care UK have considerable experience in supporting people moving from long stay hospitals or from a family home and will work with service users, their relatives/friends and other relevant agencies throughout the transition process of moving into their own home. We recognise that this can be a stressful and confusing time and will assist individuals to understand and make the necessary choices and will support them to see these through.
Each individual will have their own support worker who will get to know them and their likes, dislikes, the people that care about them, how they like to be communicated with, the kind of lifestyle they want and will work with them to develop and deliver a person centred plan. We offer varying levels of support depending on each person’ individual need so they can take an active role in running their own home and participating in the life of their local community.
