Care tailored to your loved one
We will carry out an in-depth needs assessment which will involve the individual in need of care, as well as their relatives or representative and any other professionals involved in their welfare. If your loved ones’ care is commissioned through the Borough of Poole, this will be completed by the Local Authority, Health or Primary Care Trust.
We encourage personal choice in all aspects of our care planning to maximise the potential and individual competence of all those we care for. This means supporting individuals to do as much as they are able or willing to do for themselves and to provide assistance in those areas in which they have identified needs.
We will assign members of our support team based on your loved ones’ individual needs according to their individual care plan. Our domiciliary support team will deliver a flexible, consistent and reliable service that will meet the outcomes identified in the needs assessment.
Helping people stay independent
At Poole Domiciliary Care, our main aim is to support individuals to stay as active, healthy, safe and happy as possible in their own homes. Care is tailored to each person’s needs, but may include light household tasks and personal care, such as help with washing and bathing.
We understand that many older people may require further assistance to manage medical conditions, and our care team are able to help with administering medication. Our care team receive specialist training in good care practice and supporting those living with Alzheimer’s and other forms of dementia. We are also able to accompany individuals to appointments, such as with G.Ps, dentists, and other health professionals.
