The Role

  • Partnering our discharge coordinators, you will ensure that admissions and discharges are seamless
  • Ensure all clinical assessments are completed within 24 hours of referrals, to minimise bed blocking within local hospitals
  • Ensure referred patients are suitable for our rehabilitation programme and colleagues are fully briefed on their needs.
  • Oversee the implementation of care plans, making certain we deliver the highest standards of clinical support and outstanding patient-centred services
  • Supporting residents with a range of complex needs and dependency levels

About You

  • An NMC Registered Nurse
  • Understands clinical assessment tools like Waterlow and MUST
  • Experienced in writing comprehensive care plans
